Atlas ProCare ("Atlas," "we," "us") helps Georgia families provide paid, at-home care to loved ones through the Structured Family Caregiving program. This policy explains what information we collect through www.atlasprocare.com, how we use it, and the choices you have.

This policy covers our website only. If you become an Atlas member or caregiver, the health information we handle as part of your services is protected under HIPAA and described in our Notice of Privacy Practices. If the two documents ever conflict about your health information, the Notice of Privacy Practices controls.

We collect only what you type into our forms, plus basic technical information your browser sends when you visit.

• Contact details:your name, phone number and email address

• Eligibility details (patient qualification and caregiver forms): your relationship to the person who needs care, whether you live together, Georgia county, age group, whether you work outside the home, Medicaid status and waiver enrollment

• Referral details(community partners): your name, organization and contact details, and the referred family's name, phone number, county and Medicaid status

• Messages you send us through a form or by email, including resumes sent to careers@atlasprocare.com

• Email sign-ups:your email address, if you ask to receive updates

• Survey responsesfrom current members and caregivers, if you complete our satisfaction survey online

Please don't send medical details. We don't need diagnoses, medical conditions or medications through our website. If a form has a notes box, please leave those details out; we'll talk through them privately by phone. We treat Medicaid status and waiver enrollment as confidential and use them only to check eligibility.

Information collected automatically. Like most websites, our host and the services that power the site receive standard technical data when you visit, such as your IP address, browser type, device type, and the pages you view.

• Check whether you or your loved one may qualify for Structured Family Caregiving, and help you through enrollment

• Follow up with families referred by community partners, and let the partner know the referral was received

• Measure and improve the quality of our services (satisfaction survey)

• Send caregiver tips and updates, only if you sign up

• Review job inquiries and resumes

• Keep our website working and secure

• Meet legal and Medicaid program requirements

By submitting a form, you agree that Atlas may contact you at the phone number and email you provide about your inquiry. We do not use automated or prerecorded calls.

We do not sell or rent your personal information, and we don't share it with advertisers. We share it only in these cases:

• Service providers that run the site for us, under agreements to protect it: our website platform, GoDaddy, which hosts our site and processes our contact and email sign-up forms, and our secure online form provider. They may use your information only to provide their service to Atlas.

• Program partners, when needed to determine eligibility or deliver services you asked about, such as Georgia's Department of Community Health, case management agencies and other Elderly & Disabled Waiver Program partners.

• The partner who referred you, limited to confirming that we received the referral and are in touch.

• Legal reasons: when required by law, subpoena or court order, or to protect the safety of a person.

• Business changes: if Atlas merges with or is acquired by another organization, your information would transfer under the same protections.

• Cookies. Our website platform, GoDaddy, uses cookies and similar technology that the site needs to work, to remember your choices and to measure basic site traffic. You can block or delete cookies in your browser settings, though some parts of the site may not work without them.

• Spam protection.Our contact form uses Google reCAPTCHA, which collects information about your device and how you use the form to tell people from automated bots. Its use is covered by Google's Privacy Policy and Terms of Service.

• Online forms.Our form provider may use its own cookies and security checks to keep forms working and secure.

• Analytics.We don't currently use Google Analytics or advertising cookies. If we add a tool like that, we'll update this policy first.

Our website uses an encrypted (HTTPS) connection. Form submissions are stored in secure systems that only authorized Atlas staff can access. No website or email system is perfectly secure, so please don't send sensitive medical details through our forms or by email.

We keep website form submissions for up to 6 years, as needed to respond to you and to meet Medicaid program and legal record-keeping requirements. After that, we securely delete them.

• Emails: if you sign up for caregiver tips, every email will include an unsubscribe link. You can also email us to be removed.

• Calls: tell us anytime that you'd like us to stop contacting you, and we will.

• Your information: you can ask us what information we have from your website submissions, ask us to correct it, or ask us to delete it, unless we're required to keep it. Contact us using the details below.

• Members: for your health records and HIPAA rights, see our Notice of Privacy Practices.

Our website is meant for adults. We don't knowingly collect information from children under 13. If you think a child has sent us information, contact us and we'll delete it.

Our site links to other organizations, such as Georgia state agencies and Google reviews. Their privacy practices are their own, so please review their policies.

We may update this policy as our website or services change. We'll post the new version here and update the effective date at the top.
